Care Coordinator II

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Assist with care management activities and support assigned member teams to guarantee provider services, continuity of care, and member satisfaction.

• Engage in member outreach via telephone and home visits.

• Review care plans/service plans, discuss next steps, resources, questions, concerns, and provide ongoing education.

• Coordinate care activities with healthcare and community providers, partners, members, and caregivers.

• Address inquiries, requests, and concerns from both members and providers.

• Communicate with care managers, practitioners, and others to enhance member services.

• Assist with service assessments and screenings, and document member care needs.

• Maintain and document member records, distributing them to providers as necessary.

• Identify member needs and refer to Care Managers, community-based organizations, and Disease Managers.

• Educate members on available benefits and resources.

• Adhere to policies, standards, contractual obligations, and regulatory guidelines.

• Carry out additional duties as assigned.

⛳️ Requirements

• High School diploma or GED is required.

• 1–2 years of relevant experience.

• Capability to work remotely from anywhere within the state of Missouri.

• Availability to work Monday–Friday, from 8 am to 5 pm.

• Competence in conducting member outreach through phone calls and home visits.

• Proven ability to coordinate care with healthcare and community providers, partners, members, and caregivers.

• Skill in documenting and maintaining member records in compliance with state and regulatory requirements.

• Ability to adhere to contractual, regulatory, and organizational policies and standards.

• Proficiency in identifying needs and referring to Care Managers, community-based organizations, and Disease Managers.
