This is a fully remote position, open to applicants in Missouri.
• Assist in care management functions and designated member teams
• Conduct outreach to members via phone or home visits
• Record care and service plans along with related activities
• Organize care activities in collaboration with healthcare providers, community organizations, partners, members, and caregivers
• Address inquiries, requests, and concerns from members and providers
• Communicate with care managers, practitioners, and others to enhance member services and ensure continuity of care
• Assist with service assessments and screenings, documenting members’ care requirements
• Maintain and share member records with providers as necessary
• Educate members about available benefits and resources
• Execute additional tasks as assigned and adhere to all policies and standards
• High School diploma or GED is mandatory
• 1–2 years of relevant experience is required
• Capability to work remotely from any location within the state of Missouri
• Availability from Monday to Friday, 8am to 5pm
• Ability to conduct telephonic outreach and home visits to members
• Proficiency in documenting and maintaining member records in compliance with state and regulatory standards
• Ability to adhere to contractual stipulations, regulatory guidelines, and standards
• Skill in identifying needs and referring to Care Managers, community organizations, and Disease Managers
