Care Coordinator II

This is a fully remote position, open to applicants in California.

📋 Description

• Assist in care management tasks and collaborate with assigned care teams

• Conduct member outreach through phone calls or home visits

• Engage in discussions regarding care plans, service plans, next steps, resources, questions, and concerns with members

• Coordinate care activities among healthcare and community providers, partners, members, and caregivers

• Address member and provider inquiries and concerns related to care plans and service plans

• Communicate with care managers, practitioners, and others to promote member services and ensure continuity of care

• Assist with service assessments and screenings, documenting member care needs

• Maintain member records in compliance with state and regulatory requirements

• Adhere to contractual, regulatory, and organizational standards and policies

• Identify needs and provide referrals to Care Managers, community-based organizations, and Disease Managers

• Educate members about available benefits and resources

• Undertake additional duties as required

⛳️ Requirements

• High School diploma or GED

• 1–2 years of related experience

• Preference for candidates residing in California

• Preferred ability to work PST hours

• Prior experience in healthcare is preferred

• Customer service experience is preferred

• An Associate degree is preferred
