This is a fully remote position, open to applicants in California.

📋 Description

• Evaluate and screen members to identify and provide suitable interventions and referrals.

• Act as the initial point of contact for members, providers, and medical groups regarding the administration of benefits/programs and troubleshooting access difficulties.

• Utilize motivational interviewing techniques to comprehend and address member goals and concerns.

• Organize care and services by leveraging member benefits and community resources.

• Tackle and resolve access-to-care issues to enhance health outcomes and member experience.

• Complete precise and timely assessments, care plans, service plans, and case notes across multiple computer systems.

• Submit documentation within designated timeframes while adhering to departmental procedures and training materials.

• Employ critical thinking skills to troubleshoot issues with members, providers, and vendors.

• Collaborate with county offices and governmental agencies as necessary.

• Demonstrate professionalism and empathy during interactions with members and providers.

• Carry out responsibilities under direct supervision, managing basic issues and referring complex matters to senior staff.

• Support SCAN’s Vision and Goals.

• Undertake additional duties as assigned.


⛳️ Requirements

• High School Diploma is required; some college education is preferred.

• 0-3 years of experience in healthcare or customer service.

• Capability to apply motivational interviewing techniques to accurately understand and address member concerns.

• Ability to thrive in an environment where ongoing coaching and feedback are standard practices.

• Fundamental interpersonal skills, including strong written and verbal communication abilities.

• Basic organizational skills.

• Introductory critical thinking skills.

• Ability to collaborate effectively within a multidisciplinary team.

• Capacity to maintain confidentiality appropriately.

• General understanding of CMS and DHCS regulations.

• Familiarity with medical terminology and abbreviations.

• General knowledge of local community resources available for seniors.

• Proficiency in Spanish is strongly preferred: conversational (oral), reading, and writing skills.

• Required skills in Microsoft Office (Word, Excel, PowerPoint, Outlook, Teams).

• A background check is mandatory.
