This is a fully remote position, open to applicants in California.

📋 Description

• Conduct assessments and screenings to identify and provide suitable interventions and referrals.

• Serve as the initial triage point for members, providers, and medical groups regarding the administration of benefits/programs and troubleshooting access challenges.

• Utilize motivational interviewing techniques to comprehend and address member goals and concerns.

• Organize care and services leveraging member benefits and community resources.

• Tackle and resolve access-to-care challenges to enhance health outcomes and improve member experience.

• Ensure timely and precise documentation, encompassing assessments, care plans, service plans, and case notes across various computer systems.

• Collaborate with county offices and governmental agencies to coordinate care and services as required.

• Apply critical thinking to resolve issues involving members, providers, and vendors.

• Display professionalism and empathy in interactions with both members and provider partners.

• Adhere to department procedures, workflows, job aids, and training materials.

• Escalate complex issues to higher-level staff while managing basic issues under direct supervision.

• Support SCAN’s Vision and Goals and undertake additional duties as assigned.


⛳️ Requirements

• High School Diploma is mandatory.

• 0-3 years of experience in healthcare or customer service.

• Proficiency in using motivational interviewing techniques to effectively understand and address member concerns.

• Capability to thrive in an environment where continuous coaching and feedback are standard practices.

• Fundamental interpersonal skills, including outstanding written and verbal communication abilities.

• Basic organizational competencies.

• Essential critical thinking skills.

• Ability to work collaboratively within a multidisciplinary team.

• Capability to maintain confidentiality appropriately.

• General knowledge of CMS and DHCS regulations.

• Basic understanding of medical terminology and abbreviations.

• Familiarity with local community resources for seniors.

• Proficient in Microsoft Office applications (Word, Excel, PowerPoint, Outlook, Teams) is required.

• Conversational Spanish skills in oral, reading, and writing are strongly preferred.

• A background check is mandatory.
