Care Coordinator – Coffee, Warren Counties

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Collaborate with members and their families to determine necessary supports and direct services for health, employment, and independent or community living objectives.

• Work alongside clinical and social support teams to address members’ physical, behavioral health, and long-term service requirements.

• Perform comprehensive, objective face-to-face visits in home or community settings.

• Evaluate members’ physical, behavioral, functional, psychosocial, financial, employment, and independent living needs.

• Implement authorization criteria for suitable home and community-based services.

• Ensure that authorized services are delivered and that members' needs are satisfied.

• Travel to members’ residences for visits.

• Handle requests and demands from both internal and external stakeholders.

• Employ critical thinking, problem-solving, time management, and prioritization in daily tasks.

⛳️ Requirements

• Minimum of 3 years of experience in a clinical environment.

• At least 2 years of clinical experience is mandatory.

• Must be a registered nurse with an active license in Tennessee, or hold an active license in the employee’s state of residence if participating in the Nurse Licensure Compact Act.

• Alternatively, possess a Master’s in Social Work with an active unrestricted LCSW, LMSW, or LAPSW license.

• Demonstrated exceptional customer service skills.

• Residency requirement within Coffee or Warren County, Tennessee.

• Must be available for an 8:00am–5:00pm EST schedule without on-call duties.

• Valid driver’s license is required.

• TB Skin Test is necessary for coordinators operating in the field.

• Proficiency in basic Microsoft Office applications and email.

• Strong time management abilities.

• Excellent oral and written communication skills.

• Strong interpersonal and organizational capabilities.

• Various immunizations and/or related medical tests may be necessary.

• Completion of a digital literacy assessment is required.

• Employees who regularly operate a company, personal, or rental vehicle must adhere to the BCBST Driver Safety Program, including maintaining a valid driver’s license, auto insurance compliance, and an acceptable motor vehicle record.
