Care Coordinator

This is a fully remote position, open to applicants in Canada.

📋 Description

• Act as the initial point of contact for patients and their families reaching out to Sailor Health.

• Assist older adults and caregivers throughout the intake process.

• Offer patient support, coordinate care, and provide emotional guidance.

• Aid patients in navigating their care journey, including connecting them with suitable therapists.

• Ensure that patients feel supported, understood, and confident during the entire process.

• Oversee multiple patients at various stages while ensuring accurate follow-through.

• Acquire knowledge and utilize Google Workspace, EHR platforms, and CRM tools.

• Take the initiative and contribute to shaping essential processes within a small, agile team.

⛳️ Requirements

• Located in Canada.

• Possess empathetic and clear communication skills.

• Demonstrate active listening and the ability to support individuals during challenging or emotional discussions.

• Highly organized with strong attention to detail.

• Capable of managing multiple patients at different stages.

• Comfortable with learning Google Workspace, EHR platforms such as Healthie, and CRMs like HubSpot.

• Quick to learn new software systems.

• Keen interest in enhancing mental health care access for older adults.

• Experience in healthcare, customer service, or administrative support is a plus but not mandatory.

• Must be comfortable working Eastern Time Zone hours.

• Willingness to work one of the specified shifts: 9a–6p ET or 12p–9p ET.
