Care Coordinator

This is a fully remote position, open to applicants in Maryland.

📋 Description

• Collaborate closely with the care management team to deliver short-term care coordination and connect members with necessary resources.

• Provide administrative assistance to members of the care management team.

• Gather data for Health Risk Assessment screenings.

• Evaluate members for eligibility, benefits, prioritization for care management services, and suitable referrals.

• Execute transition-of-care responsibilities by reaching out to PCPs, Medical POAs, specialists, home health agencies, and other providers.

• Generate and send routine care management letters and educational materials.

• Document encounters and communications; complete monthly reports; and maintain comprehensive electronic member records.

• Encourage members to actively engage in their health and wellness.

• Provide education and follow-up, schedule PCP visits, and conduct care coordination under the guidance of the Care Manager.

• Aid members in accessing medical homes, medical care, social services, and other health-related services.

• Coordinate resources to tackle barriers and social determinants of health, address care gaps, and ensure appropriate access to care.

• Follow up with members and providers regarding health and social services.

• Confirm appointments and obtain medical records to enhance member compliance.

• Increase awareness of community resources and services.

• Facilitate access to housing, food, clothing, prenatal and parenting classes, life-skills education, and mental health services.

• Promote communication and coordinate services among providers and members.

• Uphold the confidentiality and security of member records in accordance with policies and privacy regulations.

• Foster positive working relationships with members, providers, nurse care managers, social workers, agency representatives, and office staff.

⛳️ Requirements

• High school diploma or equivalent qualification.

• Valid State Driver’s License is required.

• Preferred: Two or more years of experience working with the Medicaid population in Maryland or Washington, D.C.

• Excellent interpersonal, communication, and customer service skills.

• Ability to work effectively with a diverse range of constituents in a community setting.

• Familiarity with community agencies and resources.

• Understanding of medical terminology.

• Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health services.

• Awareness of transportation and other barriers to accessing care.

• Ability to convey medical information to healthcare professionals and members of the care management team.

• Capable of creative and analytical thinking.

• Strong written and verbal communication skills.

• Excellent problem-solving, organizational, and time management abilities.

• Proficient in computer use and Microsoft Office applications preferred.

• Understanding of remote work tools and platforms (Microsoft Teams, Zoom, SharePoint, etc.).
