Care Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Engage with patients through multiple communication channels (text, phone) to enhance program participation, deliver reminders, and follow up on care.

• Ensure high levels of patient engagement and satisfaction by providing continuous support and education.

• Update patient information within Electronic Health Records (EHRs) and convey care plans to referring clinic partners.

• Maintain a collaborative care registry to monitor patient follow-up and clinical outcomes.

• Prepare and submit regular progress reports in the EHR.

• Facilitate external referrals for community-based social services when clinically necessary.

• Escalate crisis situations to licensed clinical team members in accordance with established protocols.

• Write, prepare, and communicate prompt responses to inquiries from patients and providers.

• Report directly to the Lead Care Coordinator and provide consistent updates on patient engagement and clinical outcomes.

• Work in partnership with OB clinic staff to ensure coordinated care and support for patients.

⛳️ Requirements

• Required: Bachelor’s degree and/or Master’s Degree in nursing, social work, or psychology.

• Clinical licensure is not required for this position.

• Strong knowledge of women’s mental health conditions, treatment options, and community resources.

• Excellent communication, organizational, and problem-solving abilities.

• Comfortable managing a high-volume caseload of patients with varying levels of acuity.

• Capability to engage and educate patients with compassion and support.

• Strong collaboration skills across departments, including Care Operations and Partner Success.

• Proficient in utilizing EHR systems and maintaining precise patient records.

• Proficient in Google Suite applications.

• Respectful of diverse backgrounds and experiences, particularly regarding perinatal and menopause contexts.
