This is a fully remote position, open to applicants in United States.
β’ Assist with the enrollment and onboarding of patients in chronic care management.
β’ Confirm patient eligibility and finalize consent documentation.
β’ Foster patient engagement and address issues with inactive or unengaged patients.
β’ Execute weekly outreach via text and phone to enrolled CCM patients.
β’ Adhere to engagement-time standards set by Medicare CCM billing guidelines.
β’ Support provider-directed care plans and identify barriers to adherence.
β’ Conduct a high volume of outbound calls, texts, and follow-up outreach.
β’ Record patient interactions in both CCM and EHR systems.
β’ Monitor engagement time and keep documentation ready for audits.
β’ Ensure compliance with HIPAA and protect PHI.
β’ Assist in facilitating access to referring providers and clinical staff.
β’ Aid in care transitions following hospitalizations or emergency department visits.
β’ Organize referrals and follow-up appointments.
β’ Refer symptom concerns or medication issues to licensed clinical personnel.
β’ Oversee patient messaging workflows and collaborate with remote clinical and operational teams.
β’ Certification as a Medical Assistant.
β’ 1β3 years of experience in healthcare.
β’ Proficient clinical assessment, communication, and documentation abilities.
β’ Comfortable using EHR and CCM platforms concurrently.
β’ Capability to manage a structured, metric-driven workflow reliably.
β’ Intermediate skills in MS Office Suite, including Outlook and Teams.
β’ Ability to work primarily from home.
β’ Home internet connection with a minimum of 25 Mbps download and 10 Mbps upload speed.
β’ Dedicated workspace free from interruptions to safeguard PHI/HIPAA information.
β’ Experience in chronic disease or nephrology is preferred.
β’ Background in chronic disease management is preferred.
β’ Prior experience in virtual care, remote nursing, or telehealth programs is preferred.
β’ Bilingual in Spanish and English is preferred.
β’ Knowledge of CCM regulatory requirements is preferred.
β’ Strong skills in patient education and motivational interviewing are preferred.
β’ Availability to work from 8:00 am to 5:00 pm Central Time.
