Care Coordinator

This is a fully remote position, open to applicants in Philippines.

📋 Description

• Collaborate closely with a team of Nurse Practitioners to deliver intraday support and coordination.

• Organize and prioritize the daily tasks of the NP team, manage schedules, track pending tasks, and provide operational assistance on a day-to-day basis.

• Monitor charts during the review process, including those awaiting lab results.

• Inform NPs when charts are ready for finalization and submission.

• Examine charts post-visits to ensure that identified care gaps have been addressed and documented in accordance with company SOPs.

• Validate osteoporosis management, eye/retinal examinations, A1C testing and results, and controlled blood pressure protocols.

• Communicate any identified issues, such as an A1C of 7.5 or above, to the NP to ensure the member receives the necessary education and follow-up guidance.

• Identify urgent concerns arising from visits and promptly notify the NP.

• Assist in coordinating timely member outreach when necessary.

• Undertake additional tasks that enhance clinical quality, facilitate chart completion, and improve NP team performance.

⛳️ Requirements

• Active clinical background.

• Valid US RN license along with clinical RN experience and background.

• Exceptional attention to detail and the ability to review clinical documentation for thoroughness and accuracy.

• Capability to manage multiple charts and cases concurrently while prioritizing tasks in a fast-paced, deadline-oriented environment.

• Strong communication skills to effectively coordinate with Nurse Practitioners throughout the day.

• Proficient in working with EMR systems and internal tracking tools.
