Care Concierge Coordinator

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver compassionate and individualized member advocacy throughout each member's therapy journey.

• Assist members in understanding their options, overcoming barriers, and advocating for necessary adjustments and services in care.

• Oversee new and ongoing therapy cases with precise documentation.

• Collaborate with providers, pharmacies, and internal teams.

• Track therapy progress and prioritize the needs of members.

• Manage inbound and outbound communications and casework efficiently.

• Address inquiries promptly and accurately using critical thinking skills.

• Foresee challenges and adapt to the evolving needs of members.

• Maintain open, proactive communication with members, providers, and internal teams.

• Cultivate strong relationships and ensure member satisfaction through professional and empathetic interactions.

⛳️ Requirements

• Minimum of 3 years of experience in the healthcare sector, specifically in care coordination, member advocacy, or similar roles.

• At least 3 years of customer service experience; case management experience in healthcare is preferred.

• Demonstrated experience in a high-volume, fast-paced, and dynamic team environment.

• Strong problem-solving skills, with a positive attitude towards adapting to changing working conditions and multitasking.

• Passionate about enhancing healthcare and supporting individuals, with a successful track record in a related position.

• Outstanding communication skills and the ability to quickly grasp and convey the benefits of the VIVIO Health program.

• Demonstrates responsibility, reliability, and accountability for work quality and performance improvement.

• High attention to detail.

• High school diploma or GED required.

• Availability for full-time work between 11:00 AM–8:00 PM ET or 9:00 AM–5:30 PM ET.

• Capable of consistently attending 3 weeks of training: 2 weeks virtual and 1 week in-person in Hayward, CA.

• Willingness to travel onsite for training and collaboration in Hayward, CA.

• Bilingual candidates are a plus.
