This is a fully remote position, open to applicants in Arizona, +2 more states.
β’ Provide care for patients with stage 3 and 4 heart failure, ensuring adherence to medication, monitoring dietary and lifestyle habits, conducting regular follow-ups, and coordinating care.
β’ Implement evidence-based practices to minimize avoidable hospital admissions and enhance quality of life.
β’ Collaborate with cardiologists, specialty physicians, field nurses, community health workers, and clinical care teams.
β’ Train, mentor, and support local Advanced Practice Providers (APPs) who care for cardiac patients in their homes.
β’ Create training programs focused on recognizing cardiac conditions, escalation criteria, medication management, patient education, care coordination, and documentation standards.
β’ Work alongside cardiology leadership and local clinical teams to identify educational needs and encourage evidence-based protocols.
β’ Perform patient assessments both in-home and virtually.
β’ Educate and counsel patients and their families about health programs and available benefits.
β’ Document patient histories, examinations, medications, allergies, assessments, plans, and other necessary records.
β’ Ensure timely completion of all documentation and paperwork.
β’ Prescribe medications, order diagnostic tests, and collaborate with cardiologists and physicians.
β’ Measure and report daily improvements in patient quality outcomes.
β’ Assist patients with enrollment in educational video programs.
β’ Participate in integrated care team meetings.
β’ Develop, implement, follow up on, and assess treatment plans.
β’ Active and unrestricted Registered Nurse and Nurse Practitioner license in Arizona, California, or Washington, or the ability and willingness to obtain multiple state licenses.
β’ Board certification from ANCC or AANP.
β’ Current and unrestricted DEA certificate.
β’ Minimum of two (2) years of experience as a Nurse Practitioner in inpatient or outpatient cardiology settings; new graduates need not apply.
β’ Ability to work independently without direct supervision and practice autonomously.
β’ Access to reliable transportation, a valid driver's license, and car insurance.
β’ Proficiency with medical instruments and equipment.
β’ Familiarity with computer-based data management systems, information systems, medical records, and point-of-interview technology.
β’ Strong verbal and written communication skills with retail and medical partners, patients, family members, physicians, and community representatives.
β’ Knowledge of federal and state regulations, including HIPAA and OSHA.
β’ Experience in patient assessment, diagnosis, treatment, and management of cardiac diseases.
β’ Preferred experience in educating, coaching, or precepting APPs or clinical teams on cardiac disease management.
