Call Center Counselor

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Assist individuals receiving care by managing transitions throughout the care system.

• Act as a primary point of contact for callers.

• Evaluate needs in the areas of mental health, medical, and social services.

• Conduct needs assessments and utilize other relevant tools.

• Initiate and submit referrals to medical and behavioral health care services, community programs, and other providers.

• Track progress towards personalized goals.

• Ensure timely access to services, including residential admissions when suitable.

• Collaborate with individuals, care teams, community partners, and multidisciplinary groups.

• Engage in supervision and team meetings.

• Maintain an up-to-date understanding of care partners, service providers, and system stakeholders.

• Aid in residential admissions via the bed registry and coordinate with external stakeholders.

• Adhere to program, department, and company policies while completing assigned responsibilities.

⛳️ Requirements

• High School Diploma, GED, or equivalent is required.

• A college degree in Human Services or a related field is strongly preferred.

• One year of work experience engaging with individuals facing dynamic and complex behavioral health challenges is preferred.

• One year of experience in community health or lived experience is preferred.

• Familiarity with person-centered, evidence-based practices.

• Capability to assess mental health, medical, social, and other service needs.

• Ability to initiate referrals to medical and behavioral health care, community programs, and service providers.

• Competence in collaborating with multidisciplinary care teams and community stakeholders.

• Availability for shifts from Tuesday-Saturday or Sunday-Thursday, from 11:00 PM to 7:00 AM.

• No travel is required.
