This is a fully remote position, open to applicants in Colorado.

📋 Description

• Evaluate patients' medical issues and concerns via phone.

• Gather patient history and triage information to identify the suitable care pathway.

• Support the management of patient interactions within the healthcare system.

• Educate patients about their healthcare needs, community resources, and managed-care obligations.

• Provide telephone guidance in accordance with established protocols and guidelines.

• Schedule provider evaluations through appointments or follow-up calls.

• Instruct patients and families by phone on minor ailments, medication usage, health maintenance, blood sugar monitoring, nebulizer application, prescribed regimens, operational procedures, and specialist referrals.

• Arrange for interpreting services when necessary.

• Engage in Quality Assurance activities.

• Work collaboratively with staff to comply with HRSA, AAAHC, OSHA, regulatory, and funding requirements.

• Thoroughly document patient care in the electronic health record.

• Ensure a safe, comfortable, and therapeutic environment for patients and families.

• Oversee patient tasks and patient/provider follow-up according to nursing task guidelines.


⛳️ Requirements

• Bilingual proficiency in Spanish and English is essential.

• Only candidates residing in Colorado will be considered for this position.

• Applicants must live within one hour's travel of our clinics.

• RN licensure from an accredited institution is required.

• A minimum of two years of experience in community health is preferred.

• Familiarity with electronic health records is preferred.

• Must demonstrate sensitivity to low-income and ethnic minority communities.

• Strong critical thinking skills are necessary.

• Proficient computer skills are required.

• Ability to thrive in a team management environment.

• Must interact with colleagues, team members, and all staff in a professional and collegial manner.

• A valid Colorado RN license is mandatory.

• Must receive the annual influenza (flu) vaccine and tuberculosis screening upon hire.

• Proof of required immunizations must be provided before employment commences.
