Bilingual Spanish Telephonic RN Nurse Case Manager – Special Needs Plan, California RN Required

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver telephonic case management for members with complex medical needs and chronic illnesses.

• Perform thorough health assessments and develop personalized care plans.

• Collaborate with both internal and external partners, including healthcare providers and specialists, to coordinate care.

• Provide education to members and caregivers regarding disease management and preventive healthcare.

• Track member progress and advocate for timely and appropriate interventions.

• Identify and assist in resolving any service or access issues that may affect the quality of care.

⛳️ Requirements

• Active and unrestricted RN license in California (Non-Compact).

• A minimum of 2 years of clinical nursing experience is required.

• At least 1 year of experience in case management.

• Bilingual proficiency in English and Spanish is essential.

• Must have proficiency in Microsoft Office applications (Word, Excel, Outlook).
