RN Case Manager, Bilingual (English/Spanish)

This is a fully remote position, open to applicants in United States.

📋 Description

• The RN Case Manager is essential in enhancing patient care and maximizing resource efficiency.

• Responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of comprehensive care plans tailored to meet individual health requirements.

• Emphasizes achieving quality patient outcomes while delivering cost-effective healthcare.

• Operates within the boundaries of nursing licensure and complies with policies, procedures, regulations, URAC standards, and state-specific regulations, making informed decisions to ensure compliance, appropriate care levels, and patient safety.

• Acts as a liaison to promote a collaborative process involving patients, families/caregivers, physicians, hospital discharge planners, home care providers, and other ancillary service providers.

⛳️ Requirements

• A Bachelor’s degree in Nursing is preferred.

• Must be a Registered Nurse (RN) with a current, active state license in good standing in the relevant state(s) where job responsibilities are conducted.

• Case management certification is required.

• A minimum of four (4) years of clinical practice experience in at least one of the following areas: case management, home health, critical care, medical/surgical, discharge planning, concurrent review, or obstetric/neonatal care.
