Bilingual Spanish Health Coach, Educator

This is a fully remote position, open to applicants in California.

📋 Description

• Perform home and virtual visits, including telehealth and telephonic consultations.

• Establish a wellness coaching relationship with patients, caregivers, and family members.

• Work together with advanced practice clinicians and/or social workers to create an action plan.

• Promote the adoption of habits that lead to an enhanced quality of life.

• Help patients in accessing community resources and social services.

⛳️ Requirements

• At least 1 year of experience as a wellness or health coach or educator.

• High School Diploma or GED is required.

• Capability to communicate positively, professionally, and effectively with others.

• Strong understanding of specific diseases, lifestyle-related topics, nutrition, and exercise.

• Familiarity with and experience in applying the stages of change or a comparable behavior change model.

• Proficient in computer skills, including Microsoft Office applications such as Excel, Word, and PowerPoint.

• Possess a current and valid California Driver's License along with reliable transportation.
