This is a fully remote position, open to applicants in Texas.
• Provide real-time clinical support and triage for newly re-engaged patients, ensuring prompt response to clinical needs following outreach and enrollment interactions.
• Offer proactive, professional, and compassionate care, acting as an accessible first point of contact for patients and caregivers.
• Utilize a family-centered care approach, meeting patients where they are and accommodating individualized care requirements.
• Exhibit cultural competence and sensitivity, effectively supporting diverse populations and utilizing appropriate resources as necessary.
• Establish trusting relationships with patients and families to foster engagement, comprehend needs, and promote ongoing participation in services.
• Conduct a thorough assessment of patients’ clinical, psychosocial, discharge planning, and financial needs, setting appropriate clinical milestones and goals.
• Collaborate with physicians, APPs, and interdisciplinary team members to create and implement care plans that align with patient needs and clinical objectives.
• Coordinate with Resource specialists, Behavioral Health, and other interdisciplinary partners to address complex needs and ensure suitable referrals and follow-up.
• Monitor and evaluate the effectiveness of care plans, collecting input from relevant sources to guarantee that care is delivered accurately, safely, timely, and cost-effectively.
• Accurately document all assessments, interventions, and patient interactions in a timely manner.
• Achieve established monthly re-engagement productivity and engagement goals, including completing patient encounters, facilitating effective warm handoffs, and maintaining caregiver engagement.
• Contribute to longitudinal engagement outcomes by assisting a target percentage of re-engaged patients to complete a clinical visit within three months of initial re-engagement.
• Must be a licensed RN in at least one state with the eligibility to register for licensure in other states.
• Bachelor’s degree in nursing from an accredited university is preferred.
• A minimum of 1 year of experience in care coordination or case management is required.
• Familiarity with Medicaid regulations and services is required.
• Experience in Value Based Care is highly preferred.
• Virtual care experience is a plus.
• Bilingual in Spanish is required.
