Bilingual Pediatric Registered Nurse

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Act as the primary liaison for families through regular virtual interactions.

• Deliver professional, friendly, and proactive support for clinical concerns.

• Integrate a family-centered care approach into service delivery.

• Exhibit cultural competence and sensitivity while engaging with diverse populations.

• Manage care transitions for ED/IP/UC patients with clinical providers post-discharge.

• Conduct thorough assessments of clinical, psychosocial, discharge planning, and financial requirements.

• Set clinical benchmarks and objectives.

• Foster relationships with patients and families, comprehend their needs and expectations, and help establish realistic mutual goals.

• Create comprehensive care plans in collaboration with physicians, patients, and the interdisciplinary team.

• Relay care plans and any modifications to patients, families, physicians, and healthcare team members.

• Collect data to assess the effectiveness of care plans and ensure accurate, safe, timely, and cost-efficient care.

• Document care management evaluations and interventions.

• Refer intricate psychosocial and discharge planning matters to Social Workers or Behavioral Health and ensure appropriate follow-up.

• Collaborate with interdisciplinary team members, including dietary and pharmacy, to facilitate safe discharges.

• Perform additional responsibilities as required.

⛳️ Requirements

• Must be a Licensed RN in at least one state, with eligibility to register for licensure in other states.

• A Bachelor’s degree in nursing from an accredited institution is mandatory.

• A minimum of 5 years of experience in pediatrics is required.

• At least 1 year of experience in care coordination or case management is preferred.

• Bilingual in Spanish is required.

• Familiarity with Medicaid regulations and services is advantageous.

• Experience with Value Based Care (VBC) is a plus.

• Experience in virtual care is beneficial.

• Nurses must be physically employed in the United States.

• CST availability is essential; the anticipated schedule is 3x12s, either Monday-Wednesday or Wednesday-Friday, from 7:00 AM to 7:00 PM CST.
