Bilingual Pediatric Registered Nurse

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Act as the main liaison for families through regular virtual interactions.

• Offer professional, friendly, and proactive care, addressing clinical concerns promptly.

• Integrate a family-centered care approach into the delivery of services.

• Exhibit cultural competence and sensitivity when engaging with diverse populations.

• Facilitate care transitions for Emergency Department, Inpatient, and Urgent Care settings in collaboration with clinical providers post-discharge.

• Conduct thorough assessments of patients’ clinical, psychosocial, discharge planning, and financial requirements.

• Set clinical milestones and establish achievable goals.

• Foster relationships with patients and families to comprehend their needs and expectations while aiding in the development of realistic shared goals.

• Create comprehensive care plans in collaboration with physicians, patients, and the interdisciplinary team.

• Communicate care plans and any modifications to patients, families, physicians, and healthcare team members.

• Collect data to assess the effectiveness of care plans and guarantee safe, timely, accurate, and cost-efficient care.

• Record care management assessments and interventions.

• Refer complex psychosocial and discharge planning concerns to Social Work or Behavioral Health, ensuring follow-up is conducted.

• Consult with interdisciplinary team members, including dietary and pharmacy, to facilitate safe discharges.

• Perform additional responsibilities as assigned.

⛳️ Requirements

• Must hold a valid RN license in at least one state and be eligible for licensure in additional states.

• A Bachelor's degree in nursing from an accredited institution is mandatory.

• A minimum of 5 years of experience in pediatrics is required.

• At least 1 year of experience in care coordination or case management is preferred.

• Bilingual in Spanish is necessary.

• Experience in outpatient settings (primary care and/or subspecialty), home health, complex care, pediatric ICU, emergency medicine, etc., is highly preferred.

• Knowledge of Medicaid regulations and services is advantageous.

• Experience with Value-Based Care (VBC) is a plus.

• Experience in virtual care is beneficial.

• Must be physically present and working in the United States.

• CST availability is required.
