Bilingual Non-Clinical Care Coach

This is a fully remote position, open to applicants in United States.

📋 Description

• Initiate phone outreach, primarily using an auto-dialer, to engage members who have multiple chronic conditions along with financial or functional challenges.

• Evaluate member needs through data analysis, structured assessments, and direct conversations.

• Create and oversee personalized care plans.

• Coordinate services and link members to suitable clinical and community resources.

• Identify and address obstacles that hinder effective care and member autonomy.

• Track progress towards desired health outcomes and modify care strategies as needed.

• Ensure strict compliance with regulatory requirements.

• Manage multiple computer systems concurrently while interacting with members in a structured setting with limited schedule flexibility.

⛳️ Requirements

• Must be a resident of Puerto Rico.

• An associate degree in any discipline; ADN, RN, or LPN degrees are not acceptable.

• Bilingual proficiency in English and Spanish, encompassing reading, writing, and speaking skills.

• A minimum of 2 years of experience in direct adult patient care.

• Experience in long-term care, home and community-based services, hospital/facility settings, case management, or managed care organizations.

• Familiarity with working alongside members of a healthcare team.

• Proficient in navigating electronic software applications, including electronic medical records.

• Proven experience in building relationships with both internal and external customers.

• Minimum internet download speed of 25 Mbps and upload speed of 10 Mbps for home internet access.

• Ability to work from a dedicated space without interruptions to safeguard member PHI/HIPAA information.

• Occasional travel to Humana offices for training or meetings may be necessary.
