This is a fully remote position, open to applicants in California.
• Contact members regarding referrals, prior authorizations, medication refills, and scheduling appointments.
• Facilitate transitions of care by performing tasks and assessments following hospital or skilled nursing facility (SNF) discharges.
• Collaborate with RN Case Managers to oversee care for a group of SNP members.
• Monitor alerts (such as new diagnoses, admissions, and discharges) and relay important changes to the care team.
• Act as a reliable point of contact for members, fostering relationships and advocating for their care.
• Ensure accurate and timely documentation within the Case Management system.
• Assist with provider communications, which includes managing record requests and distributing documents.
• Engage in team meetings and case reviews to enhance member care.
• Uphold compliance with HIPAA regulations, organizational policies, and departmental deadlines.
• Bilingual in Spanish or Vietnamese (mandatory).
• Minimum of 1 year of experience in a healthcare IPA setting.
• At least 1 year of experience with referrals and prior authorizations.
• High school diploma or GED required (Bachelor’s degree preferred; or 4 years of equivalent experience in lieu of a degree).
• Basic proficiency in Microsoft Outlook, Word, and Excel; typing speed of at least 25 WPM.
• Excellent communication and interpersonal skills.
• Capacity to prioritize and handle multiple tasks in a fast-paced, high-volume setting.
• Knowledge of Medi-Cal and managed care plans.
