📋 Description

• Deliver compassionate, patient-focused behavioral health assistance in a virtual environment.

• Conduct thorough biopsychosocial and standardized evaluations for individuals considering program enrollment.

• Create personalized support plans with patients and set achievable goals.

• Offer community service resources and facilitate referrals.

• Manage enrollment and intake processes.

• Handle telephone inquiries regarding mental health care and available services.

• Perform safety assessments and planning via telephone or video calls.

• Collaborate with patients to foster lasting, self-directed changes that enhance health and wellness.

• Work alongside multidisciplinary team members to establish patient support objectives.

• Attend multidisciplinary patient care conferences as necessary.

• Maintain documentation of interviews, consultations, referrals, contacts, and support services.

• Finalize documentation and reporting in compliance with departmental standards and federal and state laws.

• Operate in a state of constant vigilance and safety.

• Carry out additional responsibilities as assigned.

⛳️ Requirements

• Must reside within the four-state area: Missouri, Illinois, Wisconsin, or Oklahoma.

• Master's degree in Behavioral Health or an equivalent educational background for licensing.

• Required professional license/certification relevant to the work location: Illinois — LCPC, LCSW, or LMFT; Missouri — LCSW, LMFT, or LPC; Oklahoma — LADC, LBP, LCSW, LADC-MH, LMFT, LMSW, or LPC; Wisconsin — LCSW, LMFT, LPC, MFT-TL, APSW, or LPC-IT.

• Familiarity with Epic software.

• Excellent communication abilities.

• Strong teamwork capabilities.

• Proficient in crisis intervention techniques.

• No prior experience necessary.
