This is a fully remote position, open to applicants in Florida.
• Assist the social worker in fulfilling members’ mental, behavioral, and social requirements.
• Organize therapy, psychiatry, and mental health resources in collaboration with external behavioral health services.
• Perform Social Determinants of Health screenings, offer resources, and make necessary referrals.
• Work alongside multidisciplinary care teams to synchronize physical and behavioral health needs.
• Engage in community outreach and foster partnerships with local organizations and support programs.
• Collaborate with local substance use treatment providers for evaluations and referrals.
• Deliver behavioral health and wellness education to both members and employees.
• Monitor the completion of mental health screenings and maintain documentation in the EHR.
• Take part in team huddles, care conferences, and staff meetings.
• Track and report outcomes related to behavioral health referrals and member involvement.
• Keep precise documentation of interactions and follow-ups in the EHR.
• Aid in distributing educational materials regarding behavioral health and community resources.
• Support members in crises by coordinating urgent referrals and contacting the appropriate providers or emergency services as per protocol.
• Collaborate with clinical operations and training teams to enhance behavioral health navigation workflows, forms, and processes.
• Stay informed of community resources, behavioral health trends, and best practices.
• Participate in quality improvement initiatives and offer feedback to enhance behavioral health integration within primary care.
• Execute additional responsibilities as assigned to support PeopleOne Health’s mission and objectives.
• Spanish speaking is preferred but not mandatory.
• Bachelor’s Degree in Social Work, Psychology, Counseling, or a related discipline; OR Certified Medical Assistant with a minimum of 3–5 years of relevant experience.
• For those opting for the Certified Medical Assistant alternative: relevant experience in behavioral health or primary care environments; care coordination, referral management, or patient navigation; conducting screenings such as PHQ-2, GAD-7, and SDOH tools; assisting patients with behavioral health, substance use, or social service requirements.
• 1–3 years of post-Bachelor’s experience is preferred.
• Knowledge of local community resources is essential.
• Experience in mental or behavioral health, care navigation, or community outreach is required.
• Demonstrated strong verbal and written communication skills.
• Exceptional interpersonal and customer service skills.
• Authenticity, empathy, and cultural humility when engaging with members.
• Capability to maintain composure and focus in high-pressure or crisis situations.
• Familiarity with electronic health records (EHR) is preferred.
• Adherence to strict confidentiality as per HIPAA guidelines.
• Self-sufficient, reliable, adaptable, and proactive.
• Ability to adjust to changes and a willingness to learn new processes and workflows.
• Capacity to remain in a stationary position while working remotely for prolonged periods.
• Frequent usage of a computer, keyboard, telephone, and headset is required.
• Ability to clearly and effectively communicate with patients and staff via phone and electronic messaging.
• Occasional lifting of files or office equipment up to 10 pounds.
• Close vision and the ability to adjust focus are necessary.
