Advanced Registered Nurse Practitioner, CA Licensed

This is a fully remote position, open to applicants in California.

📋 Description

• Manage and navigate complex chronic comorbidities effectively.

• Develop and execute treatment plans tailored for patients and their family caregivers.

• Collaborate with a transdisciplinary team to create individualized care plans.

• Monitor the progress of patients and modify care plans as necessary.

• Work alongside healthcare professionals to ensure comprehensive holistic care.

• Stay updated on the latest trends and advancements in geriatric care.

• Conduct screenings and assessments addressing social determinants of health and behavioral needs.

• Collaborate with the transdisciplinary team throughout the care delivery process.

• Deliver high-quality customer service consistently.

• Provide clinical and medical education to both clients and staff.

• Engage in virtual care conferencing.

• Update and maintain accurate medical records.

• Participate in quality improvement initiatives and establish best practices.

• Contribute to the strategic direction of Nolia Care services.

• Offer virtual telehealth care to senior patients with chronic conditions and their family caregivers.

⛳️ Requirements

• Minimum of two years of outpatient experience in independent private practice or a larger medical center.

• Required experience with chronic, longitudinal care models in digital health.

• Preferred experience in Family Nurse Practitioners (FNPs) and Adult Geriatric Primary Care Nurse Practitioners (AGPCNP).

• Nurse Practitioner and Registered Nurse licenses in the state of California are mandatory.

• Required licensure in WA, TX, ID, FL, AZ, and/or multistate.

• Board certification as a Nurse Practitioner in geriatric care is preferred.

• DEA certification is required.

• National board certification as a Nurse Practitioner from an accredited agency is necessary.

• Master’s level Advanced Registered Nurse Practitioner (ARNP) qualification is required.

• Additional training or experience in Geriatric care, Dementia, and Palliative specialties is desirable.

• Doctorate in Nursing is preferred.

• Strong interpersonal, leadership, and problem-solving abilities are essential.

• A strong dedication to providing exceptional, team-oriented care is required.
