
Advanced Practice Provider, Palliative Care

This is a fully remote position, open to applicants in United States.
• Conduct initial evaluations and create Palliative Plans of Care for members who are newly enrolling in the Palliative Care Program.
• Manage a longitudinal panel of the highest risk patients through regular telephonic and/or video interactions.
• Offer consultative visits that include advanced symptom management, complex care goals, disease education, prognostication, medical orders, and transitions to hospice care.
• Assess hospital admissions to identify root causes.
• Facilitate transitions of care for engaged patients.
• Recognize readmission trends and assist in developing strategies to achieve or maintain a readmission rate of 15% or lower.
• Engage in regular huddles with members of the clinical and operations teams.
• Attend weekly interprofessional team meetings.
• Perform monthly panel reviews alongside the Medical Director and Operations Manager.
• Provide support to the team during periods of rapid growth or when team members are on PTO/absent, including handling phone-line coverage, outreach calls, initial visits, and monitoring incoming consults.
• Collaborate with clinical teams and leadership across departments to ensure seamless patient care.
• Participate in Devoted Medical meetings as necessary.
• Offer rotating on-call coverage during weekends and holidays.
• Uphold the Devoted Health mission by treating members as family.
• Must hold a current, active, and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in at least one U.S. state.
• Must be willing to obtain additional state licenses as required by the organization.
• A minimum of 5 years in advanced practice, with at least 3 years in Palliative care.
• ACHPN® credential preferred, or the willingness to obtain it within the first six months of employment.
• Flexibility to work across different time zones.
• Proficient with technology and able to thrive in a fast-paced environment.
• Self-motivated individual with the capability to excel in a remote work setting.
• Excellent interpersonal and communication skills.
• Comfortable collaborating within an interprofessional care team.
• ELNEC® or EPEC® training is preferred.
• Previous ICU, oncology, or geriatrics nursing experience is preferred.
• Experience in home-based palliative care is preferred.
• Previous leadership experience in an interprofessional team is preferred.
• Experience in program development is preferred.
