This is a fully remote position, open to applicants in Florida, +11 more states.
• Evaluate both acute and non-acute clinical issues via virtual video conference or phone.
• Conduct and document physical assessments and patient histories.
• Analyze patient information to develop, document, and implement management plans.
• Deliver health education, referrals to physicians, case management referrals, and counseling for patients and families.
• Plan patient care based on knowledge of the patient population and established protocols.
• Act as a patient advocate while collecting comprehensive and focused health information.
• Collaborate with primary care providers and other medical professionals to report health data and ensure adherence to guidelines.
• Utilize telemedicine to achieve the best possible patient outcomes.
• Work alongside on-call primary care providers to support anticipated clinical results.
• Execute protocols and assess progress towards expected outcomes.
• Improve telehealth practices and systems through effectiveness and success metrics.
• Communicate through various formats, tools, and technologies to provide care across the continuum.
• Offer exceptional customer service and professionalism.
• Engage in clinical rounds, conferences, risk and quality management initiatives, and relevant meetings.
• Comply with departmental objectives, performance standards, regulatory requirements, quality-care benchmarks, policies, and procedures.
• Operate under a written or electronic practice agreement.
• Contribute to the development of telehealth policies, procedures, and protocols.
• Initiate or partake in quality improvement activities.
• Join committees that support growth.
• Provide constructive feedback on colleagues' practices to enhance patient care.
• Coordinate services with other programs.
• Carry out additional duties as assigned by management.
• Graduate from an Advanced Practice Nursing program with certification in a specific specialty area.
• Bachelor’s degree in Nursing (BSN) is required.
• Master’s degree in Nursing is mandatory.
• Board certification from AANP or ANCC is required.
• Basic Life Support (BLS) certification from the American Heart Association or American Red Cross is necessary upon hire.
• Multi-state licensure including Florida, as well as at least two licenses from VA, GA, MI, MO, OH, PA, TN, TX, IL, KY, LA, and KS.
• A minimum of 3 years of acute/primary care clinical experience is required.
• A minimum of 5 years of Nurse Practitioner experience is highly preferred.
• Experience in urgent care, primary care, or geriatric care is highly preferred.
• At least 2 years of telehealth work experience is preferred.
• Strong business acuity is essential.
• In-depth knowledge of job-related functions, practices, procedures, techniques, and methods is required.
• Familiarity with medical practices to operate independently as a certified practitioner and collaborate with licensed physicians and other providers.
• Proven track record of meeting clinical performance metrics.
• Technical proficiency to conduct telemedicine visits in accordance with state and federal regulations.
• Excellent clinical judgment and problem-solving skills are necessary.
• Ability to prioritize tasks and work effectively under pressure.
• Capability to provide constructive feedback.
• Professional communication and collaboration skills with physicians, patients, and team members.
• Ability to work effectively within a multidisciplinary team.
• Proficiency in Microsoft Office Suite, including Excel, Word, PowerPoint, and Outlook.
• Willingness to travel up to 10% for meetings and training is required.
• Availability for evening, overnight, and/or weekend shifts as needed.
• Willingness to travel to Georgia or other states for onsite observation and medical record review.
• Minimum requirement to work four holidays each calendar year.
• Fluency in both spoken and written English is essential.
• Willingness to obtain independent/autonomous Nurse Practitioner practice in applicable states.
