Advanced Practice Clinician, Care Anywhere

This is a fully remote position, open to applicants in California.

📋 Description

• Conduct in-home evaluations for Alignment members, including medication assessments and health screenings.

• Deliver patient education by aligning desired care with the most suitable healthcare solutions.

• Identify diagnoses that require evaluation in care management and proactive medical oversight.

• Collaborate and communicate with the Regional Medical Officer and other team members to formulate appropriate care plans.

• Coordinate care with primary care physicians, specialists, and ancillary providers.

• Lead a comprehensive clinical team that includes nurses, health coaches, social workers, and care coordinators.

• Offer care and support to Alignment Healthcare patients in their homes to help avert unnecessary hospitalizations, health complications, and unmanaged disease progression.

⛳️ Requirements

• Required: Master’s degree from an accredited Nurse Practitioner or Physician Assistant program.

• Active Nurse Practitioner or Physician Assistant license.

• Active RN License and Furnishing number (mandatory upon hire) for Nurse Practitioners only.

• Active Nurse Practitioner Board Certification/Physician Assistant Certification.

• NPI Number, DEA, and valid BLS certification.

• Valid Driver's License and current automobile insurance.

• Ability to communicate in a positive, professional, and effective manner with others.

• Strong problem-solving, organizational, and time management skills, with the capability to thrive in a fast-paced environment.

• Preferred: One (1) year of previous clinical or home care experience.

• Prior EMR experience is preferred.

• Experience in caring for older adult (geriatric) patients is preferred.
