Adherence Coach

This is a fully remote position, open to applicants in Puerto Rico.

📋 Description

• Evaluate assigned asynchronous patient medication videos within specified turnaround times and accurately document adherence.

• Verify correct oral medication intake and identify barriers to adherence.

• Provide tailored video feedback and coaching to reinforce medication-taking habits.

• Utilize video messaging, phone calls, and SMS to enhance patient engagement and adherence.

• Proactively reach out to patients who miss submissions or demonstrate decreasing adherence.

• Assist in re-engaging patients at risk of dropping out.

• Act as the primary support for the RN team and connect patients with suitable services.

• Reinforce care plans, medication schedules, adherence to appointments, and visits with physicians.

• Assess and document social determinants of health.

• Provide authorized community and program resources.

• Escalate medication inquiries, side effects, safety concerns, health issues, and other risks to nursing staff.

• Help with basic troubleshooting of the mobile application and adherence platform.

• Keep precise and compliant documentation.

• Achieve quality, compliance, and video engagement performance metrics.

• Engage in quality audits, team meetings, and ongoing professional development.

• Offer suggestions to enhance video engagement tactics, patient experience, and adherence processes.

⛳️ Requirements

• 3–5 years of experience in healthcare engagement, care coordination, adherence support, or a comparable role.

• Experience in direct patient interaction within healthcare environments.

• Excellent written and verbal communication abilities.

• Outstanding attention to detail.

• Comfort in reviewing patient-submitted video content.

• Capability to work independently in a remote setting.

• Proficiency in CRM systems and mobile applications.

• Strong time management skills in a queue-based workflow.

• Candidates must reside in Puerto Rico and maintain residency throughout employment.

• Must have legal authorization to work in Puerto Rico/United States without employer sponsorship.

• Visa sponsorship is not an option.

• Preferred: experience as a medical assistant, pharmacy technician, community health worker, certified health coach, or directly observed therapy worker.

• Preferred: background in digital health or remote patient monitoring.

• Preferred: experience serving Medicaid or underserved populations.

• Bilingual, with Spanish preferred.

• Familiarity with principles of motivational interviewing.
