This is a fully remote position, open to applicants in Oregon.
• Deliver physician services to the Patient Access Department to ensure prompt access to hospice care.
• Offer physician guidance and educational support to admissions staff.
• Assist in gathering additional information needed to verify patient eligibility.
• Complete all required regulatory hospice physician documentation for each admission.
• Collaborate with the Director of Patient Access Services and the Patient Access/Admissions Department.
• Review, validate, and assess patient information from medical records, referring or primary physicians, and admissions nurses.
• Establish initial and ongoing hospice eligibility using prognostic indicators, Local Coverage Determinations, coexisting conditions, and disease progression.
• Provide verbal confirmation of hospice eligibility.
• Generate written certifications and clinical narratives for patients who are admitted and eligible.
• Promptly prescribe medications to ensure comfort for newly admitted patients.
• Assess the admissions process and suggest improvements for efficiency and accuracy.
• Educate the Patient Access/Admissions and Professional Relations Departments regarding eligibility information and regulatory updates.
• Identify related/unrelated medications, treatments, and hospitalizations, documenting them in the EMR.
• Review hospital patients to determine appropriate levels of care and engage in hospital interdisciplinary groups.
• Take part in quality improvement initiatives, research, educational programs, and professional/community activities.
• Perform additional duties as assigned.
• MD or DO degree from an accredited medical institution.
• Licensed to practice medicine in the State of Florida, with a willingness to obtain licenses in additional states as necessary.
• Possess a current DEA number for Florida and any other states as requested.
• State Controlled Substances Registration in Maryland and the District of Columbia is required.
• Hold current Medicare and Medicaid Numbers.
• Maintain active BLS certification from either the American Heart Association or the Red Cross, with in-person training completed.
• Minimum of five (5) years of experience or completion of a Fellowship.
• Eligibility for malpractice coverage in accordance with the Chapters’ policy terms.
• Possess knowledge and experience in palliative medical practice and symptom management.
• Basic computer proficiency including email and word processing (Microsoft Outlook and Word or equivalent).
• Must be able to physically access any home within the organization’s service area.
• Ability to manage emotional stress while working with terminally ill patients and their families.
• Consent to drug and/or alcohol testing following a conditional offer, along with ongoing adherence to the Drug-Free Workplace Policy.
• Eligibility for Florida Care Provider Background Screening Clearinghouse for services rendered for Florida affiliates.
• Hospice and Palliative Medicine certification, ABMS board certification, and recognized ABMS field board eligibility/certification are preferred.
