This is a fully remote position, open to applicants in Oregon.
• Ensure that physician services are delivered to the Patient Access Department to enable prompt hospice access.
• Offer guidance and education to admissions staff regarding physician protocols.
• Assist in gathering additional information necessary to confirm patient eligibility.
• Complete all required regulatory documentation for hospice physicians for each admission.
• Review, verify, and assess patient information from medical records, referring or primary physicians, and admissions nurses.
• Assess initial and ongoing hospice eligibility based on prognostic indicators, Local Coverage Determinations, coexisting conditions, and disease progression.
• Provide verbal confirmation of hospice eligibility.
• Prepare written certification and clinical narratives for patients who are admitted and meet eligibility criteria.
• Prescribe medications to enhance comfort for newly admitted patients.
• Analyze the admissions process and suggest improvements for efficiency and accuracy.
• Train the Patient Access/Admissions and Professional Relations Departments on eligibility criteria and regulatory updates.
• Identify related and unrelated medications, treatments, and hospitalizations, documenting them in the EMR.
• Review hospital patients to ensure appropriate levels of care and engage in hospital interdisciplinary groups.
• Adhere to company policies regarding health, safety, security, confidentiality, regulatory standards, and fraud prevention.
• Engage in quality improvement, research, educational, community, and professional initiatives.
• MD or DO from an accredited medical institution.
• Licensed to practice medicine in Florida, with a readiness to acquire additional state licenses as necessary.
• Current DEA registration for Florida and any other states as requested.
• State Controlled Substances Registration for Maryland and the District of Columbia.
• Valid Medicare and Medicaid Numbers.
• Active BLS certification from the American Heart Association or Red Cross, obtained through in-person training.
• A minimum of five (5) years of experience or completion of a Fellowship.
• Eligibility for malpractice coverage as per the Chapter’s policy terms.
• Knowledge and experience in palliative care and symptom management.
• Basic computer skills, including proficiency in email and word processing (Microsoft Outlook and Word or equivalent).
• Capability to physically access any home within the organization’s service area.
• Ability to cope with the emotional stress associated with working with terminally ill patients and their families.
• Agreement to undergo drug and/or alcohol testing following a conditional offer.
