Virtual Physician Collaborator

This is a fully remote position, open to applicants in South Carolina.

📋 Description

• Work in partnership with Advanced Practice Providers (APPs) to deliver virtual wraparound services for patients who have experienced fractures within the last six months, particularly when they are unable to visit their primary care physician.

• Collaborate with the APP to identify and address gaps in care, ensuring that patients have access to essential services.

• Conduct regular chart reviews and facilitate monthly meetings to enhance service delivery.

• Act as a resource for APPs to maintain high standards of patient care.

• Report directly to the Associate Director.

⛳️ Requirements

• Must hold an unrestricted medical license and be a resident of South Carolina.

• Graduate of an MD/DO program from an accredited institution.

• Board certified or Board eligible.

• A minimum of 2 years of experience working collaboratively with mid-level providers.

• Required internet speed of at least 25 Mbps for downloads and 10 Mbps for uploads.

• Must work from a dedicated space free from interruptions to safeguard member PHI/HIPAA information.

• Occasional travel to Humana's offices for training or meetings may be necessary.
