Virtual Physician Collaborator

This is a fully remote position, open to applicants in Kentucky, +4 more states.

📋 Description

• Work alongside Advanced Practice Providers (APPs) to deliver virtual wraparound services for patients who have experienced fractures within the past 6 months when they are unable to visit their primary care physician.

• Partner with APPs to address gaps in care and facilitate access to essential services.

• Conduct regular chart reviews and organize monthly meetings.

• Act as a resource for APPs to guarantee high-quality patient care.

• Report directly to the Associate Director.

⛳️ Requirements

• Must possess an unrestricted medical license in KY, TN, MO, NC, TX, and IN.

• Graduate from an accredited MD/DO program.

• Board certified or Board eligible.

• Over 2 years of experience working collaboratively with mid-level providers.

• Minimum internet download speed of 25 Mbps and upload speed of 10 Mbps.

• Operate from a dedicated workspace free from ongoing interruptions to safeguard patient PHI / HIPAA information.

• Occasional travel to Humana's offices for training or meetings may be necessary.
