Virtual Clinician – Home Evaluations, Nurse Practitioner / Physician Assistant

This is a fully remote position, open to applicants in Oregon.

📋 Description

• Conduct online Video Visits lasting approximately 30 minutes with members of Medicare Advantage, Medicaid, and select commercial plans.

• Review patients' medications and medical histories during these Video Visits.

• Provide education to members regarding their health and respond to their inquiries.

• Assist in addressing gaps in care and link members to the necessary healthcare services.

• Work from a private location equipped with a reliable internet connection.

• Adhere to training, affiliation, and compliance standards related to Video Visit protocols.

• Achieve visit completion rates and capacity targets.

• Participate in regular meetings with managers, team members, and clinical staff.

• Meet operational, clinical, and member experience KPIs and metrics set by Signify Health.

• Deliver telehealth services exclusively through Video Visits.

• Utilize computers, phones, iPads, and other electronic devices while engaging remotely with members.

⛳️ Requirements

• Must be a Nurse Practitioner or Physician Assistant.

• Board certification is mandatory.

• Hold active state licenses in the areas of coverage.

• Capability to obtain and maintain multi-state licenses as required in the coverage area.

• FNP or AGNP certification is preferred for Nurse Practitioners.

• Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format is preferred.

• Availability for a full-time commitment of 40 hours per week.

• Consistently provide 320+ capacity units each month.

• Consistently complete over 220 visits per month.

• Completion of training and credentialing for all health plan clients and lines of business in primary coverage state(s).

• Regular attendance in manager, team, and clinical meetings.

• Must have broadband connectivity with both video and audio capabilities.

• A quiet environment with a neutral background is required for conducting visits.

• Ability to maintain health plan member privacy according to CVS Health guidelines.

• Strong verbal, written, and electronic communication skills are essential.

• Capability to work independently without direct supervision and practice autonomously.

• Proficiency in information management and technology is required.
