This is a fully remote position, open to applicants in Connecticut.
• Provide patient care services utilizing a remote technology platform.
• Deliver comprehensive, evidence-based care for individuals aged 18 months and older.
• Assess, diagnose, treat, and manage various health issues.
• Offer health counseling and develop disposition plans.
• Educate patients on pregnancy prevention, STI prevention, and safer sex practices.
• Evaluate patients in primary care, those with acute conditions, chronic illnesses, and those requiring transitional care.
• Provide counseling on contraceptive care and manage related medications.
• Educate patients on health maintenance and address their care inquiries.
• Record all patient interactions in an Electronic Health Record (EHR) in accordance with MinuteClinic policies and procedures.
• Coordinate patient care with both internal and external colleagues, as well as the broader patient-centered medical home.
• Collaborate effectively within a patient care team that includes Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare professionals.
• Operate independently, prioritize tasks, troubleshoot issues, take initiative, and advocate for patients and the practice.
• Commit to working three weekdays and every Saturday and Sunday during the 2:30 p.m. to 11:00 p.m. shift, totaling 40 hours weekly.
• Possess a current, unrestricted license in one of the following states: CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, WV.
• Willingness to obtain additional licensure as required.
• At least two years of medically relevant experience or an equivalent background.
• Strong verbal, written, and electronic communication skills.
• Exceptional organizational abilities and capacity to manage multiple tasks simultaneously.
• Initiative, problem-solving skills, adaptability, and flexibility.
• Capability to work autonomously without direct supervision.
• Proficient in information management and technology.
• Ability to collaborate with professional colleagues to ensure quality care.
• Depending on the market, proficiency in additional languages, including Spanish, may be necessary.
• Completion of a Master's Degree level Family Nurse Practitioner program.
• Current National Board Certification.
• State of Employment license to practice in the Advanced Practice Nurse role.
• Reliable broadband connectivity.
