Virtual Care Nurse Practitioner

This is a fully remote position, open to applicants in Connecticut, +10 more states.

📋 Description

• Provide comprehensive, evidence-based remote patient care utilizing a virtual technology platform.

• Evaluate, diagnose, treat, and manage various health issues.

• Offer health counseling and care planning for patients aged 18 months and older.

• Educate and treat patients regarding pregnancy prevention, STI prevention, and safer sexual practices.

• Assess primary care, acute care, chronic illness, and transitional care patients.

• Deliver contraceptive counseling and manage medication.

• Instruct patients on health maintenance and address their care inquiries.

• Record patient care in an Electronic Health Record (EHR) in compliance with MinuteClinic policies and procedures.

• Collaborate with internal and external colleagues and connect with the broader patient-centered medical home.

• Function as part of a patient care team that includes Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare professionals.

• Operate independently, prioritize tasks, resolve issues, take initiative, and advocate for patients and the practice.

⛳️ Requirements

• Valid, unrestricted license in one of the following states: CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, WV.

• At least two years of relevant medical experience or equivalent qualifications.

• Completion of a Master's Degree in a Family Nurse Practitioner program.

• Current National Board Certification.

• State license to practice as an Advanced Practice Nurse in the state of employment.

• Strong verbal, written, and electronic communication skills.

• Excellent organizational abilities and capability to manage multiple tasks.

• Initiative, problem-solving skills, adaptability, and flexibility.

• Ability to work independently and practice autonomously.

• Proficient in information management and technology.

• Ability to collaborate effectively with professional colleagues.

• Depending on the market, proficiency in additional languages, such as Spanish, may be required.

• Reliable broadband connectivity.

• Quiet environment with a neutral background for remote consultations.

• Commitment to maintaining patient privacy according to CVSH guidelines.

• Availability to work 40 hours per week on a day-shift schedule, including every other Saturday and Sunday from 12:00 p.m. to 8:30 p.m.
