This is a fully remote position, open to applicants in Alabama, +11 more states.
• Provide comprehensive, evidence-informed care via a remote technology platform.
• Evaluate, diagnose, treat, and manage various health conditions.
• Offer health counseling and care planning for patients aged 18 months and older.
• Assess primary care, acute, chronic, and transitional care patients.
• Deliver education and counseling on pregnancy prevention, STI prevention, safer sex practices, and contraceptive care.
• Oversee medication management and address patient care inquiries.
• Record patient care in an EHR in accordance with MinuteClinic policies and procedures.
• Collaborate on patient care with internal and external colleagues as well as the broader patient-centered medical home.
• Function within a patient care team that includes Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare professionals.
• Operate independently, prioritize tasks, resolve issues, take initiative, and advocate for both patients and the practice.
• Hold a current, unrestricted license in AL, AR, CO, IA, ID, IN, LA, MI, MS, NE, NV, OK, TN, or TX.
• Have at least two years of relevant medical experience or its equivalent.
• Successfully completed a Master’s Degree level Family Nurse Practitioner program.
• Possess current National Board Certification.
• Have a State of Employment license to practice in the Advanced Practice Nurse role.
• Demonstrate effective verbal, written, and electronic communication skills.
• Exhibit exceptional organizational skills and the ability to handle multiple tasks simultaneously.
• Show initiative, problem-solving skills, adaptability, and flexibility.
• Capable of working independently and practicing autonomously.
• Proficient in information management and technology.
• Ability to collaborate effectively with professional colleagues.
• Depending on the market, proficiency in additional languages, including Spanish, may be required.
• Access to broadband connectivity.
• Ability to work in a quiet setting with a neutral background for remote consultations.
• Commitment to maintaining patient privacy per CVSH guidelines.
• Availability to work overnight, from 9:30 p.m. to 6:00 a.m., 40 hours per week, including every other Saturday and Sunday.
