This is a fully remote position, open to applicants in New York.
• Provide pharmaceutical care services to patients as part of the site Therapy Management program.
• Conduct phone-based evaluations for disease-state management and drug therapy education based on the medication risk level and established clinic protocols.
• Identify, resolve, and prevent medication-related issues while addressing barriers to medication adherence.
• Assess medication orders for accuracy, appropriateness, and safety.
• Offer medication counseling to patients.
• Collaborate with healthcare professionals to create and oversee medication therapy plans.
• Carry out patient-care assessments and discover opportunities to enhance drug therapy.
• Engage in interdisciplinary collaboration and provide clinical recommendations.
• Monitor and manage drug interactions, adverse effects, and therapeutic outcomes.
• Keep accurate patient records and document interventions appropriately.
• Stay updated with advancements in pharmacotherapy and contribute to the development of clinical protocols.
• Ensure adherence to state and federal regulations as well as accreditation standards.
• Assist in training and mentoring new hires, pharmacy students, and pharmacy technicians.
• Candidates must possess a Doctor of Pharmacy (PharmD) degree.
• Candidates must be licensed as a pharmacist in New York.
• Extensive clinical knowledge pertinent to the site’s Therapy Management program, particularly in diabetes management and/or weight control.
• Strong clinical expertise and experience in a healthcare or hospital environment.
• Exceptional communication and interpersonal abilities.
• Capacity to work effectively in a collaborative healthcare team.
• Attention to detail and a strong commitment to patient safety.
• Dedication to ongoing education and professional growth.
• Proficient computer skills are essential.
• Familiarity with Microsoft Office applications, including Outlook, Teams, Word, Excel, and PowerPoint.
• Ability to navigate the internet and utilize web-based platforms.
• Data entry and computer skills will be assessed through a pre-employment evaluation.
