Telephonic RN Case Manager, SNP – Bilingual Preferred

This is a fully remote position, open to applicants in California.

📋 Description

• Facilitates care by acting as a key resource for the member, their family, and their healthcare provider.

• Guarantees timely access to suitable care for members facing urgent or immediate situations.

• Conducts thorough assessments within their professional scope.

• Works in partnership with the member, the primary care physician, and other care team members to execute a care plan.

• Offers education and support for self-management tailored to the member’s individual learning preferences.

⛳️ Requirements

• At least 2 years of clinical experience.

• A minimum of 1 year of experience in case management.

• RN license in California (Non-Compact).

• Strong understanding of community resources and treatment alternatives.

• Proficient in PC-based software applications, including Word, Excel, and PowerPoint.
