This is a fully remote position, open to applicants in United States.
• Evaluates and examines the medical, behavioral, and/or vocational condition of the injured worker, employing Carisk’s patient-centered, strengths-based biopsychosocial methodology.
• Aids in formulating a Care Coordination plan aimed at enabling the patient to achieve optimal functional enhancement.
• Interacts with the patient, existing and prospective providers, as well as vocational and social support networks and other external stakeholders.
• Collaborates with Carisk’s Clinical, Quality, Provider Relations, and additional internal teams.
• Oversees a patient’s journey towards the intended outcomes through a customer-facing monthly progress report.
• Coordinates referrals, consultations, therapeutic services, and collaborates with other specialists regarding care and treatment strategies.
• Diligently documents all cases, activities, and hours in line with customer-specific protocols and consistent with Carisk’s documentation standards.
• Participates in all meetings and conference calls as requested by Carisk's Clinical and Administrative teams.
• Maintains all necessary credentials and complies with all ethical standards associated with those credentials.
• Executes various professional responsibilities as assigned by management.
• Current, active, and unrestricted Nursing License (RN or LPN) is required.
• Preferably, 1-2 years of experience in Workers’ Compensation and Case Management.
• Certification as a Case Manager (CCM) is preferred.
• Bilingual proficiency in English and Spanish is mandatory.
• Availability to work from 11:30am to 8:00pm EST is required.
