This is a fully remote position, open to applicants in Tennessee.
• Coach, mentor, supervise, direct, and assess Support Coordination professionals.
• Evaluate staff performance, service quality, productivity, compliance, and utilization metrics.
• Facilitate or coordinate training and development opportunities for staff.
• Ensure effective, person-centered support coordination activities for ECF CHOICES members.
• Oversee the creation and implementation of person-centered support plans, focusing on employment and integrated community-living outcomes and goals.
• Authorize services described in support plans and track progress towards established goals.
• Provide immediate guidance, performance coaching, issue resolution, operational support, communication, and mentorship.
• Ensure compliance with ECF CHOICES guidelines, policies, procedures, as well as TennCare and BlueCare standards.
• Occasionally manage a member caseload in response to census changes or staffing needs.
• Cultivate relationships with members, families, providers, and community partners.
• Assist with timely assessments, visit completions, critical incident follow-ups, and service coordination.
• Travel is required for 90% of the workday.
• Sponsorship is not offered for this position.
• RN with a valid license in Tennessee, or an active license in the employee’s state of residence if participating in the Nurse Licensure Compact Act; or a Master’s in Social Work with an active unrestricted license; or a Bachelor's degree in social work, nursing, education, or a related field approved by TENNCARE; or meet federal criteria for a Qualified Intellectual or Developmental Disabilities Professional (QDDP/QIDP).
• Minimum of 3 years of supervisory experience.
• At least 2 years of experience supporting individuals with intellectual and developmental disabilities.
• Proficient in Microsoft Office applications (Outlook, Word, Excel, and PowerPoint).
• Demonstrated analytical and problem-solving skills, with the ability to perform complex analytical tasks.
• Proven leadership capabilities that foster employee growth and development.
• Exceptional oral and written communication skills.
• Strong interpersonal and organizational abilities.
• Possess a valid driver’s license.
• Must adhere to BCBST Driver Safety Program requirements when operating a company, personal, or rental vehicle, including maintaining auto insurance and an acceptable driving record.
• Various immunizations and/or associated medical tests may be necessary.
• Remote work arrangement available.
• Occasional travel may be necessary.
