SUD Nurse Practitioner

This is a fully remote position, open to applicants in Arizona.

📋 Description

• Perform health assessments to evaluate substance use, co-occurring conditions, and overall well-being.

• Identify health issues and develop tailored treatment strategies.

• Prescribe and oversee medications, monitor for adverse reactions, and modify treatment plans as necessary.

• Collaborate with the Holon care team and outside providers to ensure coordinated, holistic care.

• Act as a clinical consultant for complex or high-need cases.

• Provide education to clients and their families about treatment plans, medications, and recovery support methods.

• Keep precise and timely records in accordance with ethical, legal, and medical standards.

• Engage in team meetings, professional growth opportunities, and ongoing training.

• Deliver telehealth assessments, medication management, and care coordination for individuals with substance use disorders (SUD).

⛳️ Requirements

• Current and unrestricted Nurse Practitioner (NP) license in Arizona.

• Active DEA license (and CDS where applicable).

• Several years of experience in behavioral health, addiction medicine, or psychiatry.

• Background in addiction medicine or psychiatry.

• Board certification in specialty preferred.

• Strong skills in documentation.

• Proficient in electronic health record (EHR) platforms.

• Compassionate attitude toward individuals dealing with SUD and mental health issues.

• Ability to work collaboratively in integrated and team-oriented care environments.

• Organized, adaptable, and capable of navigating complex systems.

• Clear and confident communication skills for virtual interactions.

• Availability for a full-time commitment of 40 hours/week or part-time commitment of 20 hours/week, based on arrangement.
