Service Coordinator I

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Evaluate, identify, secure, and oversee services for program participants tailored to their individual requirements.

• Address participant needs through various communication channels.

• Adhere to compliance standards and required timelines for assessments, service plans, visits, calls, and incident reporting.

• Record each visit and interaction in the necessary systems in a thorough, person-centered manner.

• Offer training and assistance to participants and consumer employees who are managing direct care services.

• Maintain and use internal databases to monitor participant information and interactions.

• Assist participants in acquiring accessibility modifications, specialized medical equipment, non-medical transportation, and other necessary services.

• Exemplify company values in all interactions and responsibilities.

⛳️ Requirements

• Bachelor’s degree REQUIRED; preferred fields include social work, psychology, or related areas.

• OR a minimum of three years in a social services or healthcare-related environment REQUIRED.

• Experience with individuals with disabilities or seniors is preferred.

• A minimum of one year of experience in LTSS, Service Coordination, or Case Management is preferred.

• Exceptional communication, customer service orientation, integrity, job knowledge, reliability, problem-solving abilities, interpersonal skills, productivity, quality, commitment to self-development, and teamwork.

• Proficient in computer systems and technologies pertinent to the position.

• Capability to perform reaching, stretching, stooping, sitting, walking, and standing.

• Ability to independently lift up to 20 pounds.

• Valid driver’s license and reliable transportation are required.

• Willingness to travel throughout Center County and neighboring counties.

• Practicum experience is a bonus.
