This is a fully remote position, open to applicants in New York.
• Execute care management responsibilities for individuals with complex/high-acuity medical and physical health needs.
• Perform UAS Assessments for members enrolled in the MLTC program with Fidelis.
• Assess service requirements and propose plans to achieve optimal outcomes.
• Create and continually evaluate long-term care and service strategies.
• Work in partnership with care management teams to identify healthcare providers, specialists, and community resources.
• Coordinate with members, their families/caregivers, and care provider teams to ensure a person-centered approach to care.
• Monitor member conditions, complications, symptoms, progress of care plans, and any changes in health status.
• Analyze member data, referrals, and intake assessments to enhance care quality and operational performance.
• Collaborate with healthcare professionals to facilitate services and treatments while adjusting care plans as necessary.
• Gather, document, and maintain member data and records of care management efforts.
• Inform members and their families/caregivers about disease processes, care gaps, provider instructions, available care options, referrals, and healthcare benefits.
• Serve as a liaison and advocate among members, families, physicians, facilities, and agencies.
• Organize community resources and service authorizations for services including meals, employment, housing, foster care, transportation, and daily living activities.
• Conduct home and/or other site evaluations as needed.
• Collaborate with leadership to enhance care quality and deliver cost-effective services.
• May mentor and train new clinical hires, assist with onboarding, and evaluate readiness for competency.
• Undertake additional responsibilities as assigned while adhering to all policies and standards.
• Reside in either Orange or Rockland County, New York.
• Current NY RN Licensure is required, or NP with active state nursing licensure.
• Must be a graduate of an Accredited School of Nursing OR possess a Bachelor's degree along with 4–6 years of relevant experience.
• A Bachelor's degree in Nursing is preferred.
• Experience in conducting UAS Assessments for MLTC members is necessary.
• Familiarity with care management, long-term care planning, service coordination, and healthcare benefits is essential.
• Capability to evaluate complex/high-acuity populations and coordinate medical and supportive services.
• Ability to perform home and/or other site visits.
• Must adhere to state, federal, and clinical guidelines.
• Proficiency in educating members and their families/caregivers on disease processes, care options, referrals, and available benefits.
