This is a fully remote position, open to applicants in Florida.
• Assess the appropriateness of pharmacotherapy for high-risk patients with chronic illnesses such as diabetes, hypertension, heart failure, and COPD.
• Implement clinical protocols authorized by the organization and state regulatory bodies.
• Enhance drug coverage benefits by utilizing formulary products, cost-effective alternatives, and home delivery pharmacy services.
• Collaborate with physicians, nurses, and other healthcare professionals to improve patient care.
• Engage in multidisciplinary high-risk rounds and modify treatment plans as necessary.
• Provide patient counseling and convey intricate medical information to both patients and providers.
• Instruct patients on disease management, medication usage, and self-care practices.
• Take part in medication safety and efficacy quality improvement initiatives.
• Conduct post-discharge medication reconciliation for high-risk transition-of-care patients.
• Record clinical activities in the clinic's Electronic Medical Record (EMR) and necessary pharmacy or clinical management systems.
• Assist in achieving CMS Stars and HEDIS quality metrics.
• Maintain accurate documentation of clinical interventions and patient care activities.
• Provide in-service education and training for healthcare personnel.
• Create and share educational materials on pharmacotherapy.
• Stay updated with advancements in pharmacotherapy and clinical guidelines.
• Advise healthcare providers on complex medication management challenges.
• Offer support to the Primary Care Organization within the pharmacist's area of expertise.
• Execute targeted initiatives and priorities as directed by Pharmacy leadership.
• Report to the Associate Director Integrated Clinic Pharmacist.
• Support the full physician panel across health plan coverage and potentially various clinic locations.
• A current and unrestricted pharmacist license in the state of Florida.
• Willingness to obtain licensure in additional states for market support.
• Pharm.D. or an equivalent advanced pharmacy degree.
• At least five (5) years of experience as a practicing pharmacist.
• Experience in a team-oriented environment and in achieving shared goals.
• Proficient in Microsoft Word, Excel, PowerPoint, Teams, and Outlook.
• Capability to troubleshoot and resolve basic technical issues in a remote setting.
• Clinical residency in ambulatory care or a related field is preferred.
• Board certification in geriatric pharmacy is preferred.
• Experience in an ambulatory care or primary care practice setting is preferred.
• Familiarity with electronic medical record (EMR) systems is preferred.
• Experience managing medication therapy for patients with chronic conditions is preferred.
• Ability to identify and implement innovative solutions for enhanced efficiencies and outcomes.
• Multilingual capabilities, preferably in English and Spanish.
• Must reside within a 50-mile radius of Englewood Park, Orange City, West Oakland, West Orange, or Plant City.
• Availability to work Monday through Friday, from 8:00 a.m. to 5:00 p.m. Eastern Time.
• Willingness to travel approximately 5%, typically once per quarter for market meetings.
• Access to home internet with a minimum of 25 Mbps download and 10 Mbps upload speed.
