This is a fully remote position, open to applicants in United States.
• Perform proactive outreach and follow up with members post-hospitalization.
• Link members to behavioral health services for better support.
• Organize appointments and facilitate access to benefits and community resources.
• Establish trusted relationships with members to evaluate their needs and identify suitable providers and resources.
• Tackle barriers to care and escalate risk issues to clinical teams as necessary.
• Collaborate with clinicians, Provider Success, and internal teams for follow-ups on high-risk members, provider access, program enrollment, and continuity of care.
• Inform members, providers, and facilities about available services and care management procedures.
• Address access obstacles and close care gaps effectively.
• Ensure precise and timely documentation while managing sensitive member information in accordance with HIPAA and organizational standards.
• Efficiently navigate multiple systems, communicate clearly, meet performance targets, and contribute to a team-oriented, member-centered environment.
• Bachelor's degree in Social Work, Psychology, or a related healthcare field; OR a high school diploma with 4+ years of relevant experience in healthcare, managed care, behavioral health, and/or customer service.
• At least 2 years of experience in a healthcare, managed care, behavioral health, and/or customer service setting for candidates with a bachelor’s degree.
• Strong computer skills, including the ability to work across multiple systems and accurately enter or retrieve data.
• Capability to speak and type at the same time while navigating systems during real-time member interactions.
• Familiarity with behavioral health or medical terminology.
• Excellent problem-solving, critical thinking, and organizational abilities.
• Competency in prioritizing, planning, and multi-tasking while managing competing priorities.
• Must pass a background check upon hiring and throughout employment, which includes criminal, employment, education, sanctions, prohibited-party, sex-offender-registry, and 10-panel drug-screen checks.
• Experience in autism services is preferred.
• Access to high-speed internet with a minimum of 20 Mbps download, 5 Mbps upload, and a maximum latency of 100 milliseconds, set up before starting.
• Frequent use of computer and phone systems is required.
• Ability to remain stationary, communicate and exchange information, inspect data, perform repetitive motions, interpret information, problem-solve, make decisions, organize, plan, and maintain a positive and professional demeanor.
