RN Care Manager

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Collaborate closely with patients, families, caregivers, and healthcare providers to ensure the delivery of targeted, organized, coordinated, cost-effective, and patient-centered care.

• Work alongside interdisciplinary teams and stakeholders throughout the care continuum.

• Foster optimal health for patients by implementing disease prevention and management strategies.

• Utilize evidence-based interventions to serve populations, ensuring cost-efficient and outcome-driven care.

• Focus on disease-related quality measures, patient satisfaction indicators, access to services, and rates of hospital and emergency department utilization.

⛳️ Requirements

• Bachelor’s Degree (Mandatory)

• APRN - Advanced Practice Registered Nurse License from the State of Ohio Board of Nursing

• BLS - Basic Life Support certification from the American Heart Association

• RN - Registered Nurse license from the Ohio Board of Nursing
