Resource Consultant

This is a fully remote position, open to applicants in Virginia, +1 more state.

📋 Description

• Provide guidance, mentorship, education, and support to individuals, the elderly, and those with disabilities who self-direct Personal Options services through West Virginia waiver programs.

• Conduct home visits with participants at least every six months.

• Exhibit proficiency in Personal Options services, relevant West Virginia waiver programs, and Palco policies.

• Inform and mentor participants and/or their designated representatives regarding program rights, responsibilities, and associated risks.

• Assess and monitor participants' ability to engage in the program.

• Assist in the creation and execution of initial and updated service/spending plans.

• Track the utilization of service/spending plans.

• Address and report complaints and grievances related to FMS and Resource Consulting services.

• Serve as a mandatory reporter for suspected abuse, neglect, exploitation, and other incidents as required.

• Employ a person-centered approach in interactions with participants.

• Record participant visits using a company-issued laptop.

• Log events and generate reports electronically.

• Carry out daily administrative responsibilities, which include scheduling or confirming visits and responding to company emails and calls.

• Adhere to Palco's operational procedures.

• Engage in shadowing new resource consultants and attend periodic training sessions.

• Timely disseminate new information to assigned participants.

• Manage company equipment and data in accordance with company policies and HIPAA privacy regulations.

⛳️ Requirements

• A minimum of three years of experience in a public-facing role is required.

• Preferred experience includes roles such as patient navigator, community connector, community health worker, lay health advisor, health educator, or case manager.

• High School Diploma or equivalent is required.

• Must demonstrate professional verbal and written communication skills.

• Ability to document visits electronically is essential.

• Proficient skills in Microsoft Office, Microsoft Outlook, and Internet use are required.

• Good problem-solving abilities are necessary.

• Willingness to travel within a designated geographic area to meet participants in their homes is required.

• Must be open to working with individuals of all ages and disabilities.

• A valid driver's license and reliable transportation are mandatory.

• Must have and maintain an active prepaid or contract cell phone plan for ongoing communication.

• Ability to pass a criminal background check and maltreatment check is required.

• Travel is a necessary component of this role.
