This is a fully remote position, open to applicants in Ohio.
β’ Coordinate the logistics of patient transfers by utilizing client-defined workflows to facilitate patient acceptance and bed placement.
β’ Determine the appropriate level of care, accepting provider, consultation, and access from hospitals, clinics, extended care facilities, and physician offices.
β’ Ensure precise clinical representation, prompt access, suitable bed assignment, and accurate documentation of patient information.
β’ Collaborate with clinicians, physicians, hospital personnel, medical staff, leadership teams, and external agencies.
β’ Employ critical thinking and clinical decision-making skills to create suitable destination care plans.
β’ Serve as a liaison among patients, physicians, hospital personnel, client facilities, and external agencies.
β’ Oversee individual and team compliance and performance metrics.
β’ Support process modifications as well as regulatory and quality initiatives.
β’ Identify, track, and report obstacles to patient access and throughput.
β’ Report safety, compliance, and operational risks through the established chain of command.
β’ Engage in process improvement, professional development, peer development, peer review, shared leadership, and quality data collection.
β’ Maintain a valid RN license in the states where transfer center services are provided.
β’ Perform additional tasks and analyses as requested or directed.
β’ Work assigned shifts on a six-week rotation, including every other weekend, on-call duties, and rotating holiday responsibilities.
β’ Complete three weeks of orientation from 8:00 a.m. to 4:30 p.m. ET and align with a preceptor during the fourth week.
β’ Active Registered Nurse (RN) license β Required.
β’ Active multistate/compact RN licensure β Required if applicable based on the state of residence.
β’ Associate Degree in Nursing or diploma (ADN) β Required.
β’ Minimum of 1 year of acute care experience β Required.
β’ Must have a designated workspace with a locked door, in accordance with HIPAA regulations.
β’ A secondary work location that meets HIPAA regulations is encouraged.
β’ Minimum internet speed of 100 Mbps download and 20 Mbps upload at both primary and secondary work locations.
β’ Proficient in EHR data entry, telephony systems, and office software.
β’ Familiarity with EMTALA, Ethics and Compliance, quality initiatives, and HIPAA regulations.
β’ Skilled in Microsoft Office tools, including Outlook, MS Teams, Excel, and Word.
β’ Ability to review and interpret patient transfer case information.
β’ Capacity to escalate and inform about changes in patient status.
β’ Ability to document tasks and conversations related to patient transfers.
β’ Demonstrated critical thinking, active listening, attention to detail, and multitasking abilities.
β’ Excellent oral and written communication skills.
β’ Bachelorβs degree in nursing (BSN) β Preferred.
β’ 3+ years of acute care experience β Preferred.
β’ Experience in a healthcare contact center β Preferred.
β’ Familiarity with Epic electronic health records β Preferred.
β’ Experience with Genesys contact centers β Preferred.
